Suggest Treatment For Runny Nose, Cough And Mild Fever

Question:
Hi doctor , my baby is 6 months old ...3 days back he was having a runny nose that cleared up yesterday by its own but from yesterday he is having cough and mild fever. Doctor prescribed him azee 200 once a day and XXXXXXX cough syrup..i wanted to know if antiobiotic are safe for such a small child. Thankx

Detailed Answer:
Hi....cough while recovering from cold is quite common. This is protective mechanism by which any accumulated phlegm might come up from the lungs. The baby might not be able to spit it out, but will swallow it. It will come out either in vomiting or motion.

I AGREE WITH YOU THAT THERE IS NO NEED FOR AN ANTIBIOTIC. WHATEVER YOU HAVE QUOTED HERE AS YOUR BABY'S HEALTH ISSUE DOESN'T WARRANT ANTIBIOTIC LIKE AZEE (Azithromycin).

Cough and cold are viral 95% of the times in children. For cold you can use anti-allergics like cetirizine and for nose block, saline nasal decongestants will do.

If fever - Paracetamol can be given in the dose of 15mg/kg/dose (max ceiling dose 500mg) every 4-6th hourly, that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

For cold you can use Cetrizine at 0.25mg/kg/dose every 12 hourly for 3 days.

For nasal block, plain saline nasal drops will do, every 4-6th hourly to relive nasal congestion.

Hello doctor.....thanks for your reply...i have few queries. First of all, doctor prescribed XXXXXXX d syrup for cough thrice a day ...so should we give that as he is having cough? Secondly he is not having any cold now it was for only two days and it was clear watery discharge from nose so can we assume its a viral infection? How do we get to know if its a viral infection or bacterial infection? Thirdly the baby is exclusively breastfed so if it is a bacterial infection ,then can it be cleared by its own with the help of breastmilk..and how much days should it take the infection to clear if its a viral infection? Its been 5 days since it started with a cold ...he is having cold and 99.5 fever which came to normal after giving XXXXXXX syrup and has no other symptoms

Hi....

1. I don't suggest XXXXXXX as it is a cough suppressant and I as I have already told you, Cough is a protective reflex at this age. It should not be suppressed.

2. Bacterial infections will not subside so early like this so its only viral. Moreover bacterial infection will manifest with fever which is high grade.

3. In bacterial infections child will be more toxic.

4. Viral illness lasts - 5-7 days.

Hello doctor ...thanks ...now he is not having any fever...if we take it from the arm then it is around 98.5 which i guess can be taken as normal after adding .5 degree..he occassionally have mild cold which is very less..now i am concern about cough which has increased from the last two days. I am not giving him any medicine ..its the 8th day today so i want to know how many days should i wait for the cough to get resolved by itself..because it is disturbing him a lot and has increased and disturbs him a lot while sleeping

It may take around 5-10 day for it to completely recover. But as I told you cough is protective reflex and it should not be treated as such.
