Suggest Treatment For Sessile Rectal Polyp

Question: I have been diagnosed with a 4cm sessile rectal polyp with benign biopsy. It has reoccurred on the same site where a small sessile polyp was removed 3 years ago during a routine colonoscopy. What is the best method for removal ?

I have gone through your query with diligence and would like you to know that I am here to help. In my opinion an endoscopic mucosal resection (EMR) would be the best, safest and most rapidly simple option for removal of a polyp the size of 4 cm.

I would also like to know when the biopsy of the current sessile polyp was conducted and if you could attach a scanned copy of the biopsy result. Was this conducted during a routine endoscopic procedure or was it advised due to the presence of symptoms?

The polyp was found after a recommended colonoscopy because of very low iron and haemoglobin for which I have had an iron infusion since this latest colonoscopy.this polyp has regrown in a 13 month period. At that time of my regular 3 yearly colonoscopy it was all clear. I dont have a copy of the biopsy results as i was informed over the phone of those results. This biopsy was done two weeks ago in a colooscopy.I am concerned that the EMR might not be possible because of scarring from the removal of the previous polyp. I am also concerned that this polyp might return as seems to be the case now. Its on the same site as a previous sessile polyp that was removed i would assume in a similar manner 3 years ago.

Hello once again ma'am.

I would first like to start by stating that many (most) polyps can go undiagnosed due to their minute size making them undetectable on a colonoscopy most of the times. So what I would like to add is that it would be extremely difficult to say whether or not the new polyps has occurred on the exact same place or not.

It could be possible that the new polyp was present in the region, but went undiagnosed and after growing appears to be present in the same location (approximately).

There have also been cases of the previous polyp not being adequately excised leading to a regrowth and the appearance of a recurrence. I would either way recommend an EMR despite the scar tissue, as this should not really be an issue.

Unfortunately ma'am, polyps can recur and in some individuals recur at a very high rate (not always pre-cancerous or cancerous). So we cannot really differ treatment or our approach just due to the recurring nature of polyps.
