You have symptoms of Dyspepsia and GERD.
Please continue taking Nexium.
The fullness you get after you get after a few bites might be due to gastroparesis.
Get your blood sugar level checked.
You will get relief with medication like Reglan.
For complete relief, the following should also be done.
1. Weight reduction if obese
2. Avoiding alcohol and smoking
3. Avoiding coffee, citrus, chocolates, carbonated beverages, tomatoes and fatty food
4. Eating multiple smaller meals and going to bed at least 2-3 hours after food
5. Following some stress relief methods like meditation
Also get a Repeat Upper GI endoscopy done to rule out ulcer disease.

Unlikely to be hiatal hernia

Hi,
Paraesophageal hernia is very rare.
Sliding hiatal hernia which is more common usually does not cause any symptom.
When it becomes symptomatic, it causes symptoms like heartburn, discomfort behind the breastbone etc.
Pain in the upper abdomen is caused by issues like peptic ulcer, pancreatitis etc.
But the one that you get when you exert yourself, stretch or twist is usually muscle or nerve related pain
Physical therapy and tylenol might provide relief.
