Question: have what i think is severe GERD. I will eat and sometimes feel sick right away and throw up. Also, I have had some fatty foods fried chicken and got severe heartburn that resulted in throwing up what appeared to be clear stomach acid. There was no partially digested food. This was not food poising or a stomach virus.

I had an endoscopy and there was no ulcers found and I tested negative for H. pylori bacteria, which is associated with stomach ulcers. The endoscopy was done a little over a year ago.

I have a Hiatal Hernia and a proclivity to getting heartburn do to my condition. The Hiatal Hernia was discovered during an endoscopy about 12 years ago or so.

Brief Answer:
Add prokinetic drug to PpI's

Detailed Answer:
Hi,
Thanks for query.

Gastroesophageal reflux disease, or GERD, is a digestive disorder that affects the lower esophageal sphincter (LES), the ring of muscle between the esophagus and stomach. Reflux means to flow back or return. Therefore, gastroesophageal reflux is the return of the stomach's contents back up into the esophagus. The backwash (reflux) irritates the lining of your esophagus and causes GERD.In normal digestion, the lower esophageal sphincter (LES) opens to allow food to pass into the stomach and closes to prevent food and acidic stomach juices from flowing back into the esophagus. Gastroesophageal reflux occurs when the XXXXXXX is weak or relaxes inappropriately, allowing the stomach's contents to flow up into the esophagus. If not treated, chronic acid reflux can injure the esophagus and, in some people, progress to a precancerous condition known as Barrett’s esophagus.

Use antacids like omeperazole or pantoprazole before meals.

Using motility agents like domperidone will help with rapid stomach emptying and decreased refurge or heart burn or vomiting.

Don't lye flat right after meals but take brisk walk or stay upright for almost half an hour after meals.

Avoid spicy, greasy, oily foods as well as fizzy drinks. Avoid too much quantity if hot beverages.

Rx

See as the name suggests ACID REFLUX should be treated in two phases-
1)Control production of acid-For which you should take pantoprazole 40 mg twice daily.
2)To prevent the acid formed inside stomach to reflux back into esophagus-I would suggest you to add Levosulpiride. Itis a prokinetic drug and is very effective in controlling reflux of acid back into esophagus.A better way is to take it with Rabeprazole trials have shown that its very effective when combined with Rabeprazole.

Thanks
