Suggest Treatment For Severe IBS While On Imodium

Question: I have been diagnosed with IBS on and off since 1999. I have had this reoccurring problem since XXXXXXX this year. Severe diarrhea 6 + times a day. Watery, now after taking Imodium 3 x a day I can leave the house. No blood, foul smelling. I have also had shingles in the past, in 2011. I am having pain in the upper left quadrant. Feels like a pea in my side. Wakes me for last 2 nights, feeling it again as I write.

Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.

Your symptoms are suggestive of Irritable Bowel Syndrome (IBS) only.

However, IBS is a diagnosis of exclusion. We investigate suspected cases of IBS with Thyroid Profile, Fasting Blood Sugar,Ultrasound Abdomen

Now, IBS is of three types:
1. Diarrhea predominant
2. Constipation predominant
3. Mixed variety.

You seems to be suffering from IBS (Diarrhea predominant).

I advice following for treatment of IBS:

1. A proton pump inhibitor like Pantoprazole along with Levosulpride, which will help in neutralizing any acid if you have in stomach.
2. You should avoid fat and pepper in diet and prefer to take soft, bland diet.
3. You can take Smooth Muscle Relaxant like Mebeverine for stomach pain, in case you have.
4. In refractory cases drugs like chlordiazepoxide and clidinium bromide can be used, if the condition is not responding.

My TSH 3-4 have back WNL. I do recall taking a proto pump inhibitors in the past. I however have no health insurance nor the funds to pay for the ultrasound.
