Brief Answer:
Most probably ureteric stone]

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Female/60 - moderate to severe abdominal pain and back pain - comes in waves - not visited a Doctor - no medications - started sort of after drinking some lemonade...
The commonest causes of pain in abdomen with back ache that comes in waves is a stone in the ureter.
Another reasons can be as per the additional history and investigations.
I would advise you the following:
Send the samples of urine for routine, microscopy, culture and sensitivity.
Tests of blood and stool.
Ultrasound of the whole abdomen.
X-ray of the abdomen.
Clinical evaluation and examination by a Doctor, preferably by a General Surgeon.
All these things will give the proper diagnosis and a plan for further symptomatic and definitive management.
I hope, this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
