Suggest Treatment For Severe Abdominal Cramps And Headaches In A Person With IBS

Question: Hi My 16 yr old daughter has been diagnosed with IBS, she is on the FODMAP diet and has also been prescribed tablets by our GP. She has missed a lot of school because of this. She gets severe cramps in her stomach and also pains in her back and headaches, she is also extremely tired. I really don't know what to do next, she has had a scope done but nothing showed up. Any suggestions would be appreciated.

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist (DM). I appreciate and can understand your concern regarding your daughter.

Few things to know about IBS:
1. Is a chronic disease which has relapsing and remitting course
2. It will never harm even if it will persist for whole life
3. Stress, life events, and anxiety leads to aggravation of symptoms
4. Intestine becomes hypersensitive so even discomfort is perceived as pain
5. Increase gas production leading to abdominal fullness and bloating
6. No alarming features like weight loss, gastrointestinal bleeding, or fever

So don't panic and be cool. Main treatment of IBS is symptomatic and diet modification. Among this as you have told that she is on FODMAP diet. So I will discuss about medications

For abdominal cramps give her antispasmodic like tablet normaxin which is a combination of chlodiazepoxide, clidinium and dicyclomine, this I found very useful in my patients with IBS with pain, dose of which you may discuss with your doctor. May give some SSRI (selective serotonin reuptake inhibitor) like sertraline which reduces hypersenstivity to pain, this also I found very useful in my patients with refractory pain. Give her cleaner drinking water. So what I suggest you to visit a gastroenterologist who will take detail history and treat accordingly. You may discuss with him about the medicines I have mentioned and get the prescription.
