Suggest Treatment For Severe And Persistent Lower Back Pain

Question: Hi, I'm 36 male that's experiencing excruciating pain in my right lower back. It started from my groin area and now the pain is sitting around my right quadrant going across my hip. It's been ther for about 5days now. Previous to this my gp prescribed me amoxicillin 7 day course which I've completed. And few days later this back pain started. I've been told before by my gp that I've got IBS in the past. I've not picked up any thing heavy to cause such pain. I'm in such agony. My gp sent me for an X-ray which I've had 2days ago and awaiting result with suspected stones.
I'm currently taking diclofenac sodium 50 mg which works a little but the pain keeps coming back.. Help!!!

I assume that you consider the pain being caused by a kidney stone? If so, you should have some problems with urinating (like pain when urinating, burning while urinating, low amount of the urine). If a stone obstructs ureter, and you wont do anything with it for a day or two, infection of the kidneys appears. It gives general symptoms of the infection like fever, weakness, sweating.
Nausea is also very common symptom of a kidney stone.

If you have some or most of this symptoms, you should not wait for the x-ray result. You should rather visit ER as soon as possible, otherwise your kidney will be in danger and your life consequently.

From the other hand, Diclophenac usually does not help with the kidney stone pain. Such pain is caused by smooth muscles contraction and the only drugs which do help are smooth muscles relaxing drugs including Buscolisine, Metamizole and Petidine.
If the Diclophenac helps, spinal pain can be the reason. It can be caused by spine disc herniation which compress the nerve. This nerve gives pain in the lower back and radiates to the groin and leg.
In such case you should have an MRI of the spine.

I haven't got a fever as such.. I've tried a plaster called belladonna plaster which helps with the pain which I've placed around the lower back.
However the pain in my groin is still constant and in the right side.
My only concern is that it might be due to my appendix which concerns me..
I've experienced the pain coming on to 7days to date. I've got no sign of infection, my urine is the same.
What could be the possiblities??
Is it spinal or am I just experiencing myalgia due to my IBS??

Spine issues seems to be the most probable reason.

Detailed Answer:
Appendicitis pain does not radiate to the back. Actually it does not radiate anywhere. You can feel it while walking (the pain due to appendicitis). And it is usually located in the lower right abdomen, not the groin. Slight fever, nausea, accelerate heart rate (over 100) are typical symptoms for appendicitis. Moreover, appendicitis does not last such long. Usually it forces patients to visit a doctor right away. Otherwise it gets much worse.
Inguinal hernia can give the pain in the groin. But again, the pain does not radiate to the back. Same thing with an IBS pain.
So the most probable reasons are either kidney stones or spine issues. Lack of fever and urinating problems makes the spine much more possible. Please take a hot bath. If it should help with the pain.
