Brief Answer:
Can be a serious problem.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/61 - severe middle back pain - radiates to abdomen...

This can be due to the following reasons:
Prolapsed inter-vertebral disc causing compression over the spinal cord or nerve roots causing the pain in abdomen
Shingles can present the same way.

Another possibility is you have intra-abdominal problem but feeling that the pain is coming from back to abdomen. These are
- Gastric ulcer on the posterior wall
- Pancreatitis
- Any disease of the posterior abdominal wall like dissecting abdominal aneurysm
- Rarely acute cardiac problems.
Hence I would advise you the following:
Rush to ER or the nearest hospital.
Get admitted for
Clinical evaluation, examination and urgent investigations
Ultrasound of abdomen- MRI of the spine and Pancreas and posterior abdominal wall
Tests of blood: Amylase, lipase, CBC, liver function tests and other relevant tests.
EKG

These will help to get an early and proper diagnosis and management.
I hope this answer helps you.
