Regarding your concern, I would like to explain that in order to relieve your dad's back pain, besides using Baclofen (that is beneficial at diminishing muscle spasticity), it is necessary to use any anti-inflammatory drugs (like Ibuprofen, Naproxen, etc.).
Also some light gradual physical exercises (like yoga, aerobics, etc.) would help furtherin relieving muscle spasticity. Warm aplications would be beneficial in this regard.
But, you should also know that using concomitantly anti-inflammatory drugs may interfere with the level of anticougulation (may increase INR level). That's why it is important to frequently check INR level and adjust Warfarin daily dose apropriately.

You should know that sweating and heating are symptoms related to the activation of the autonomic nervous system from the pain with high intensity.
They do not indicate anything else, except for a high intensity of the pain.
Is the pain triggered by body movements or respiration? This would be another argument in favor of musculo-skeletal pain.

- a shoulder X ray study (if the pain is located only in one shoulder)
