Posted on Wed, 30 Nov 2016
Question: hello, my father who is 83 has been told he has prostrate cancer that has metastasized into the bone. He has had back pain for the last 3 weeks, he had an MRI and the MRI confirmed a bulged disc, but also saw the bone deterioration. He is going to see an oncologist tomorrow. what kind of treatment is best? and IF this has gone into the bone does that mean it will eventually go into all other bones? and is there a short life expectancy?

Brief Answer:
Treatment includes castration and bone therapy

Detailed Answer:
Hello dear. I have gone through the details.

So its a stage 4 disease. The treatment will include castration either surgically (by removing the testes) or medically by suppressing male hormones by giving drug. Also he will require bisphosphonate drugs which will strengthen the bones by preventing fracture and pain.

Radiation to bones may be required later in the course. Disease can be well controlled at this stage and this treatment will prevent further spread to other areas.

Thanks and regards

Dr. Sundriyal, is it stage 4 because it has metastasized?

What drugs will suppress the male hormones? would this be considered ADT therapy?

Can he still have the radiation, my father has stints in as he had an aerotic aneurysm corrected a few years ago.

You say it can be controlled at Stage 4? so this is not a death sentence?

What about pain meds? he is on Oxycodone with Acetomenaphin and it is barely helping. Can we try medicinal marijuana?

He also only has 1 working kidney which is in Stage 3-4.

Brief Answer:
Hello. Metastasis means stage 4.

Detailed Answer:
Hello dear. Metastasis means stage 4 disease. Yes ADT will be required to controll the disease. Its ot a death sentence as disease can be well controlled. Stents are not a contraindications for radiation. I wont advise marijuana at this stage as pain can be controlled easily with other drugs.
Thanks and regards
