I read your question and i understand your concern. Ear pain not responding to medications like painkillers and antibiotics is worth evaluation . There are many causes of ear pain and the treatment of the underlying cause with help the pain. Pain killers alone wont help.

I would like you to go to an otorhinolaryngologist. Let them take a detailed thorough history, any discharge, prior history, smoking history , acid reflux history, previous ear surgeries, swimming, hygiene, foreign bodies, waxes or cerumen deposition etc.

Once the location of the cause of ear pain , whether outer middle or inner ear, is localised, treatment will then vary accordingly.
