Suggest Treatment For Severe Fever, Sore Throat And Loss Of Appetite
Question: I HAVE QUESTION REGARDING THE POSSIBILITY OF HIV IN MY CASE. I AM A HEALTHY 28 YEAR OLD LIVING IN XXXXXXX XXXXXXX
I HAD AN ONE TIME UNPROTECTED ENCOUNTER WITH AN UNKNOWN FEMALE.
1. 10 DAYS LATER: BAD THROAT- SLIGHT FEVER
2. 15 DAYS POST ENCOUNTER : HIGH FEVER: ADMITTED TO HOSPITAL WITH HIGH FEVER, DENGUE NS1 ANTIGEN POSITIVE, IGM AND IGG NEGATIVE, NO OTHER SYMPTOMS OTHER THAN SORE THROAT, FEVER AND EXTREME WEAKNESS/ LOSS OF APPETITE
3. 20 DAYS POST ENCOUNTER: DISCHARGED BUT RE- ADMITTED WITH VERY LOW PLATETELT COUNT - NO FEVER, NO BLEEDING. DOCTOR ASKED HIS JUNIOR IN FRONT OF ME HOW DENGUE NS1 ANTIGEN CAME POSITIVE BECAUSE MY SYMPTOMS DID NOT MATCH WITH DENGUE
5. DENGUE IGM – WEAKLY POSITIVE ON 23 DAYS, IGG NEGATIVE
6. HIV TEST NEGATIVE ON 21 DAYS
7. HIV TEST NEGATIVE ON 28 DAYS
8. HIV TEST NEGATIVE ON 56 DAYS
9. HIV TEST NEGATIVE AT (90 days)
10. HIV TEST NEGATIVE ON 120 days)
11. 6 MONTHS LATER:- TREATED FOR PERIANAL ABSCESS
12. SEVERE STOMACH PAIN FOR 3 MONTHS
13. HIV TEST NEGATIVE ( 8 months)
14. 6 MONTHS LATER: SURGERY FOR ANAL FISTULA
15. HIV TESTS NEGATIVE AT 10 MONTHS)
16. AFTER 9 MONTHS SEVERE BACK ACHE – CONTINUING TILL DATE
18. DID DENGUE IGG AFTER 1 YEAR - NEGATIVE
MY QUESTIONS ARE THE FOLLOWING:
1. WAS MY FEVER ACTUALLY DUE TO DENGUE I HAVE DONE DENGUE IGG AFTER 1 YEAR, IT IS NEGATIVE. ONLY NS 1 WAS POSITIVE AND 1 TIME, IGM WAS WEAKLY POSITIVE. IGG SHOULD HAVE BEEN POSITIVE AFTER 1 YEAR IN CASE I HAD DENGUE
2. SHOULD I TRUST THE HIV TESTS OR SHOULD I GO FOR RNA PCR TEST SINCE I HAVE READ REPORTS OF PEOPLE TESTING NEGATIVE ON NORMAL TESTS BUT LATER IDENTIFIED POSITIVE ON RNA PCR TEST. I CAN EVEN SEND A LINK OF THE TIMESOFINDIA REPORT WHICH SHOWED PEOPLE TESTING NEGATIVE ON ELISA BUT LATER IDENTIFIED POSITIVE BY RNA PCR
3. SHOULD I GO FOR HIV-2 DETECTION PCR
I MUST INFORM THAT ALL MY TESTS ARE FROM VERY REPUTED LABS ALL DOING THE 4TH GENERATION HIV TESTS. HOWEVER, THE PROBLEMS THAT I AM FACING IS MAKING ME RETHINK TIME AND AGAIN THAT I MAY BE THE EXCEPTION IN CASE OF HIV TEST ( FALSE NEGATIVE)

Brief Answer:
You are definitely free from HIV.
Detailed Answer:
Dear XXXXXXX
Welcome to HCM.
HIV test result after 3 months of exposure is surely accurate and conclusive as the window period for HIV also varies from 15 days to 3 months so don't worry. You need not to go for RNA PCR and it is not possible to get false positive result after 9 months.
Fever in the case can be due to any other infection not dengue. Consult any other physician to isolate the cause and follow the treatment regime as per the advice. Be relaxed, calm down and move ahead in life. For any further query get back to me again.
Thank You
