Suggest Treatment For Severe Hair Loss

Question: Hi Doctor,

I am 35 year old male with no medical history or allergy.
I had very dense hair since my childhood and have no baldness history in my family.

Recently noticed good amount of hair fall while bath and combing. Also noticed that my hair are getting thin. I have not seen any baldness yet but this hair fall is continue for quite sometime now and if it goes this way, I am sure it will lead to baldness.
I am using a branded anti dandruff shampoo and wash my head every day.

I am a gym goer and try to keep protein rich diet in my schedule. I take a US brand whey protein also as a supplement for last one year, only 30 grams a day which I feel should not be a problem. However I started taking creatine for sometime and took 5 grams a day for 20 days. I feel that can be a possible reason for this change. Though I have stopped taking creatine for last 20 days but as of now the hair fall in continue.

Please suggest if I need to take some multi vitamin or anything else which can help me saving my hair. Or do you think I should visit a Trichologists.

Hello. Thank you for writing to us

I am dr.kakkar (dermatologist). I have gone through your query and I have noted your concern.

You seem to have androgenetic alopecia. A lack of family history in family members does not exclude androgenetic alopecia.
A history of hair thinning and early balding in cousins (maternal > paternal cousins), uncles and aunts is also a factor that needs to be considered.
If I was the treating doctor I would start you on topical minoxidil 5% solution (Mintop forte solution), 1ml, twice daily. It is to be applied on dry and clean scalp, starting from the center and covering all the affected area. Minoxidil is a non-specific hair growth promoter.
In addition you must also take a tabet of finasteride 1mg (Finax tablet), once daily. Finasteride is 5 alpha reductase enzyme inhibitor which converts testosterone to dihydrotestosterone (DHT) in hair follicles. Since DHT is the main culprit behind hair thinning and hair loss therefore blocking this enzyme prevents further thinning and hair loss.
I also suggest a tablet of hair supplement once daily e.g either Revilus tablet or Keraglio Men tablet
If you also have dandruff you may use an antidandruff shampoo e.g Selsun shampoo Or Scalpe Shampoo, daily or every alternate days.

Thanks doctor,

I have started taking pills as well as Mintop Forte as suggested by you.

Couple of doubts,
Can I apply Mintop after bath on scalp with wet hair or I should avoid that.
Another thing is, I m not at all bald right now. Infact have good hair however my hair turning thin and falling that is why I asked for help. hope taking your recommended prescription is ok in that condition too.

Regarding use of minoxidil

Detailed Answer:
Hi.

Minoxidil is to be applied on dry and clean scalp. Not at all on wet hair as it would dilute the concentration of minoxidil. Air dry your hair after shampoo every morning and then apply minoxidil. At night you need not shampoo again. Apply 1ml, twice daily (morning and evening), starting from the center of scalp (to prevent trickling down) and moving outwards. Avoid hair oil while you are using minoxidil.
Yes, In fact these medications work quite well in early stages of thinning and hair loss. In late stages or cases with bald areas, transplant is the only option.
