An EKG needs to be done primarily to look for a possible cardiac cause for the chest pain. If the EKG is normal then NSAIDs and muscle relaxants can be advised for a week to see for improvement. If the symptoms persist then blood counts, ESR and imaging investigations e.g x ray chest can be advised to look for other causes. The investigations can be recommended based on the physical exam findings. The history of vague pains with feeling of fat ball pieces is not specific enough to consider a single diagnosis. GERD (Gastro esophageal Reflux disease can also present in somewhat similar way and can be treated accordingly with PPIs like pantoprazole along with pro kinetics.
