Brief Answer:
Myopathy needs to be evaluated

Detailed Answer:
Hello,
I have gone through your question and understand your concerns.
The raised cpk and proximal muscle weakness is suggestive of myopathy.
The exact diagnosis is required as some diseases are curable and some are not.
Metabolic tests like Thyroid profile, vitamin D, renal function test, liver function test, blood sugar should be done.
Is there any muscle pain, skin lesions, fever, deglutination difficulties?
Any family history of muscle disease?
Get investigated in a neurology set up.
Provide prescription reports and examination findings if available.
Hope you found the answer helpful.
Do get back to me for further queries.
