I went through your query and understood your problem.
Here I would like to advise you to first get the low back pain investigated for the cause. Many a times it could be something that can be taken care of and pain will vanish without help of medication. Sometimes it could be severe and reducing the pain will mask the underlying problem.
At your age considering underlying kidney problem it could be osteodystrophy or any vertebrae impinging upon the nerve causing the root pain. An x ray lumbar spine will help to find out any changes in the spinal structure which may be resulting in the nerve entrapment. An MRI spine should definetly pick up the spinal cord structure and any disturbance in its anatomy if present due to verterbal chages and its effects on spine.
I would advise you to consult an orthopaedic surgeon for the same. Get a x ray or if required MRI scan of the lower back to find the exact reason for the pain.
Gabapentin 100mg 3 times a day or you can try 300mg once at night only. Gabapentin has renal excretion mode so in long run it could have deleterious effects on kidney and as you have already chronic kidney disease it could worsen it. But the pain alleviating effect of gabapentin is good and does work well.
But all in all I would first ask you to investigate the cause of pain and find specific treatment for the same.
