Question: Hi doc, my girlfriend feels heavy pain in stomech whenever we get along pre sex activities to be specific. She is vergin and she got to know this few days back only. We are not sure if its normal for everyone to have sever stomach (lower abdomin) pain while getting physical, if someone is vergin or is it something we shld get treated or get checked. To summerise its more like a pain during masturbation i would say, as she hadnt had sex ever and we didnt even tried therafter. It lasts for 20 mins and then normal again and would only reappear once she would ever try to get axcited otherwise. It happens almost everytime.

Brief Answer:
likely endometriosis...needs ultrasound pelvis for diagnosis...explained.

Detailed Answer:
Hello,
Thanks for the query to hcm,
The typical pain in lower abdomen especially in the uterine area following sexual excitation causes uterine contractions and a pull on the utero-sacral ligaments and the round ligament which usually doesn't cause any pain unless there has been adhesions or endometriotic lesions due to endometriosis. The fact that the pain lasts only 20 min till the sexual excitation and uterine contractions last indicate that there are adhesions between tubes and uterine wall, likely due to endometriosis or that there may be an adnexal mass possibly an ovarian cyst or a uterine fibroid.
This calls for an ultrasonological examination of the pelvis with an aim to rule out endometriosis, ovarian cyst or adnexal mass or a uterine fibroid.
Please get the ultrasound done and review with reports.
In the meantime pain can be controlled with NSAIDS and hot fomentation.

What would be the possible treatment if ultrasound confirms any of these. What can be the consequences of long term ignorance.

Brief Answer:
long term consequence can be infertility issues.

Detailed Answer:
Hello,
if there is endometriosis then she would have to be placed on progesterone only pill for at least 6 months to control the same and if there are adhesions then she may have to undergo laparoscopic adhesioloysis . Adhesions with fallopian tubes can often cause hydrosalpinx and often cause of blocked tubes leading to infertility.
