To recapitulate: Male/22 - vomiting - pain in lower right back and around his side - ok on shallow breathes - severe stabbing pain on deep breathing - kidney stones about 5 years back ....
The possibilities as per the history you have provided are:
- Hydronephrosis i.e. fullness of collecting system of kidney on right side with/without stone and/or infection.
- But does not go with with severe stabbing pain on deep breathing a the pain in hydronephrosis can increase slightly on deep breathing, hence there is chance of infection but there is no history of fever, chills, urinary symptoms.
- Hence other chances to be ruled out are:
Lower lung/pleura and ribs inflammation.
Rarely inter-vertebral disc problems.
- Pathology of liver and/or gall bladder.
- Vomiting is usually due to reflex to pain.
I would advise him the following:
Tests of blood: CBC, liver and kidney functions.
Urine: Routine, microscopy, culture and sensitivity.
Ultrasound of the whole abdomen, lower chest for pleural fluid.
X-ray of the abdomen.
Further tests and management as per the clinical evaluation, examination and preliminary investigations.
