Question: Father woke with great tenderness on the bump (highest point on the left shoulder - clavicle?). No awareness of activity strain or bumping it privious day. No sign of trama / briusing etc. on skin. The soreness has increased for three days - now difficult to raise left arm above waist level without pain -sneezing or coughing also causes pain to left shoulder

Hi,

I am so sorry to hear about this shoulder pain that your father has. The absence of trauma makes the cause difficult to establish without examination and evaluation. I will suggest that he sees his doctor for evaluation and an x-ray. This is more so because the use of Tylenol and Ibuprofen have failed to control his pain. He will also benefit from physical therapy to avoid having reduced range of movement at this joint.

In the interim, appropriate pain control is okay. I will go with 650mg of Tylenol every 8hours, and 400mg of Ibuprofen every 8hours(after food) while pending evaluation. After seeing his doctor, he might benefit from more potent pain medications if the pain persists.
