Suggest Treatment For Sharp Shooting Pain In Abdomen And Nausea

Question: My son is currently taking a Zpack and phenagran/codeine for strep. He has been on the medication 4 days. Tonight he is having sharp shooting pains in abdomen and vomitting without feeling nauseous. Should I be concerned?

Hi Emakersey ! Good morning. I am Dr Shareef answering your query.

It is not uncommon to have pain in the abdomen with vomiting in respiratory tract infection in a child. Over and above, Azithromycin itself could cause some gastritis and vomiting specially if taken in empty stomach. I would advise your son for a proton pump inhibitor, along with an anti spasmodic and an anti emetic for a symptomatic relief for the present.

However, one should keep other possibilities of acute abdominal pain in mind, and so if the pain with vomiting did not get relieved with the above regimen or increased in intensity, I would advise your son to be clinically assessed by a general surgeon to rule out chances of any acute abdomen. Also in case he eats from outside, he should avoid it as intestinal infections could creep in from outside food which are not made very hygienically. Also a stool test for ova and cyst, and a urine routine/microscopic test along with a complete blood count and an ultrasound abdomen if need be would be an additional supporting investigation to clinical assessment.
