Brief Answer:
An orthopedic physical examination is needed.

Detailed Answer:
Hi

Thank you for asking HCM.

I have gone through your query. Your problem can be due to damage of internal structures meniscal or ligament injury. Other possible cause is tendinitis. Chondromalacia should be ruled out also. The pain occurs when locking the knee from your description.

A physical examination by an orthopedic surgeon will gives clue and a X ray followed by a MRI will help to confirm the condition. Have you done any MRI of knee recently.

Wearing knee braces will be helpful till that time. In case of meniscal or ligament injury arthroscopic correction will be helpful. In case of tendinitis local injection of corticosteroids will be helpful.

Let me know if anything not clear. I am happy to help you.
Thanks.
