Suggest Treatment For Skin Infection
Question: Hi Doctor,
I was having some kind of skin infection on my nose and nose folds which happened because of my frequent white heads removal in the saloon when consulted to doctor on this app it was said it was seborroheic dermatitis and was advised to use 0.05% desonide cream to heal the lesion and later 2% ketoconazole cream to prevent reoccurrence I used the creams as advised and it helped to heal the lesion on my nose folds but the lesion on my nose is still not healed completely please suggest. I have attached a pic for the same.

Thanks for responding doctor, I dont have acne problem as such The lesion has been around 6 months now, i had same kind of infection on nose folds as well but it got healed by the creams i mentioned in the message i didnt try to squeeze it but the infection increases whenever i roam out in dust and comes down when im not but it never gets healed completely. Its not painful.

Thanks for the additional info. I have also reviewed your previous discussion with another doctor regarding the red rash on the sides of the nose. Though you assumed it to be due to frequent white head removal but that is not the reason. It was a type of dermatitis called Seb Dermatitis which characteristically involves the sides of nose, scalp, and sometimes eyebrows and eyelashes too.
The medicines prescribed by the doctor were topical steroids which work well since this being a steroid responsive dermatoses. However, the dermatitis can relapse once you stop steroids and since steroids cannot be used forever a long time on face therefore you were asked to continue with a topical antifungal (ketoconazole cream) which is safer even for long term use.
Coming back to the lesion on top of nose, this could be either part of the same condition or it could be something else. Acne is unlikely since you say that it has been there for 6 months.
Purely based on morphology and distribution it could either be seborrheic dermatitis Or it could be a patch of Discoid Lupus Erythematosus.
What I want to know specifically from you is:
-What was the response of this patch to the medicines/creams which you used?Are you still using them?
-Has it increased or decreased in size over the last 6 months?
-Is it scaly? can you scrape some scale with your nail.
