Suggest Treatment For Sore Mouth Along With Sore Throat And Cough

default
Posted on Sat, 14 Mar 2015
Question: I have had a sore mouth for over a month. I have no visible signs of irritation such as pustules, leisions or surface abnormalities. My tongue, under my tongue, inside of lips and top of mouth all feel as if I have eaten a massive serving of pineapple or acidic food. I have tried alkaline mouth wash (soda bicarbonate) and over the counter antiseptic sprays and mouth washes. Nothing has helped. Then I developed fever, sore throat, cough etc., but believe those symptoms are related to flu, and despite those symptoms improving the soreness in my mouth has not improved. I do not have acid reflux. Problem is isolated to the burning within my mouth. Any thoughts?

Brief Answer:
Oral mucosal ulceration, Glossitis, anemia

Detailed Answer:
Dear,
welcome to HCM
From the information presented it appears a case of oral mucosal ulceration and glositis.

I need to know answers to few question
1) are you feeling sensitive to spice foods than normal?
2) is your oral mucosa red? (a photo of the lesion would be perfect here)
3) is your tongue red and painful?
4) taste appears to be altered?

Considering past infection with possible nutritional deficiency it appears likely. An underlying severe vitamin deficiencies and anemia needs to be ruled out.
Therefore It is advisable to get a Vit b12, complete blood haemogram done.

The above asked information will help to provide a better clinical picture.
Do not use any antiseptic sprays, sodium bicarbonate solution, etc.
An antibiotic course with vitamin supplements, over the counter medication under the guidance of physician is must.

Kindly followup with the answers.

Regards,
Dr Pankaj Borade

1. No increased sensitivity to spicy foods;
2. Oral mucosa is not red or signs of inflammation and no lesions;
3. Tongue is not red but burning at back of tongue, front (tip) of tongue and under tongue;
4. Taste not noticeably altered,

More medical background:
1. I take L-Thyroxine 75 MCG tabs for hypothyroidism
2. I use serum drops for severe dry eye symptoms

Last week I had blood test (I can send lab report) but noting unremarkable
I have been taking Azithromy 250 MG ZPak Tab since Saturday for flu symptoms. Flu symptoms now less severe, but burning mouth pain is unchanged. I have had the mouth pain for over a month. It came on suddenly and has persisted ever since. I do take Vit B 12 and Vit D3, COQ-10, and a Cal-Mag supplement. I have good diet. I am active, 5'6", 133 lbs.

I will avoid Mouth wash and antiseptic sprays etc as recommended...
Hope that helps with further analysis of the problem.
thank you. XXXXXXX XXXXXXX

Hope that helps.

Brief Answer:
Unlikely presentaion

Detailed Answer:
Thanks for the reply.
Sore mouth without any objective signs of inflammation and lesions is very unlikely.
Any burning or painful sensation is associated with inflammatory activity by rule.
The co occurring medical conditions appears unrelated.
To advice you,
I would suggest to continue the supplements and medications as advised to you by your treating doctor. You can definitely take up this issue with your treating consultant as one may come up with some objective signs in supporting of some condition on a detailed inspectory examniation.

Hope to have satisfied your query.
Stay healthy.

I have researched burning mouth syndrome and find nearly everything with that diagnosis to be consistent with my symptoms. I have made an appointment with an oral specialist. Apparently it is a condition that (1) is hard to diagnose, (2) not all that common, but consistent with hypothyroidism etc., (3) treatment uncertainties. Therefore I will seek help of specialist. Thank you
