Brief Answer:
It can be muscular spasm due to spondylitis.

Detailed Answer:
Hello,

I have studied your case and I think your wife is having cervical spondylitis. Pain in neck along with shoulder pain is caused by disc herniation in the neck. It leads to compression of the nerve of shoulder and it causes pain. I have following suggestion for patients with cervical spondylitis.

1. Avoid keeping neck in same position for long time and also do not use hard or soft pillows.

2. Intermittent cervical traction exercise will help. Also neck muscle strengthening exercises is useful.

3. She can take Pregabalin and Methylcobal for shoulder pain. Muscle relaxant like Myoril or Myospaz is helpful for neck pain.

4. She might need MRI of the neck if above mentioned measures are not working.

I hope these measures will be helpful in you. Let me know if there is any other follow up questions.

Thanks
