Suggest Treatment For Strep Throat And Abscess After Unprotected Anal And Vaginal Intercourse

Question: Hi, I am a male 28 years old.
On the 1st of January I had unprotected vaginal and anal intercourse with a girl. She claims to be clean of any stds. 2 days later I developed Strep throat or Tonsillitis which I have had recurring several times over the the past year. Also I felt sort of a sore throat the past couple days before I had sex with her so I think the sex is unrelated to the strep throat or Tonsillitis I got. Anyway, 28 days later I was in the emergency room of the hospital having a Perianal Abscess drained which seemed to have developed rapidly from the 24th of January to the point where It was super painful on the 28th when I had it drained. I had blood tests done while I was at Hospital (not sure what kind of tests) and was out of hospital the next day. They didn't tell me any reasons why it occurred or anything (or what kind of Bloodtests they did or any results), although the doctor asked if I was XXXXXXX when I first got admitted to the emergency room, probably wondering if I had receptive anal sex which I have never in my life and am not XXXXXXX Could I have caught something of the girl I had sex with to cause this Perianal Abscess almost a month later? We did not go ANYWHERE near my Anus so I am confused on why or how I could have got it from her if I did? I have also have been sick free this year which is like a first for me so feeling really healthy. Please explain your thoughts. Thanks

Brief Answer:
You should screen for Hiv and other STDs
Detailed Answer:
Hello. Thank you for writing to us.
I have gone through your query and I have understood your concern.
As far as unprotected sex is concerned it is risk factor for all STDs including Hiv and you should get screened for the same. An isolated perianal abscess is not a sign of any STD in particular and this abscess could be unrelated to that particular sexual encounter. Though Hiv related mmunosuppression can predispose to bacterial infections at unusual places like a perianal abscess, therefore you should get screened for Hiv and other STDs since your last sexual encounter was unprotected.

Can you please be more specific on the development time I got my perianal abscess in regards to the time from which I had unprotected sex? I mean 28 days later is unsual right? A bit too long?
Also Would they have not checked my blood or screened for any abnormalities when I was in the hospital for any STDs etc?

Brief Answer:
Hiv screening by Rapid test/ Spot test
Detailed Answer:
Hi
My only concern is to rule out Hiv. Unprotected vaginal and anal sex is considered as a high risk contact as far as Hiv infection is concerned. Hiv lowers body immunity and makes one prone to bacterial infections and abscess.
Since you developed this abscess approximately 3 weeks after unprotected sex therefore I would like you to get screened for Hiv.
You may take screening test right away i.e Spot test Or Rapid Hiv test.
The time lag of 3-4 weeks between possible Hiv infection and development of this abscess is nothing unusual.
Not sure if the hospital where you got abscess drained also screened you for Hiv unless you also mentioned about your risky sexual encounter 3-4 weeks ago. You may inquire this from them.
You should anyways have got yourself screened after that incident of unprotected sex because it was a high risk contact.
You cannot rely on her even if she said that she is clean. Protected sex would have been safer.

I have been screened and I am clean, I just want to also ask whether anal fissures could be a cause? I was noticing during that time that I had a lot of anal fissures from a very poor diet which was making my time in toilet a little painful
Brief Answer:
Anal fissure could be a cause of perianal abscess
Detailed Answer:
Hi.
Ok.
That is good if you have been screened and declared clean.
Yes, anal fissure is also one of the inciting factor for perianal abscess. Probably this is the reason the doctor who drained your perianal abscess at ER inquired if your sexual preference was gay! since he must have noticed your anal fissures.
