Brief Answer:
Brain MR and calcium blockers

Detailed Answer:
Dear
I read your query and understand your concern
I am sorry for the symptoms your husband is suffering
I read about his treatment and there are still anti hypertensive pills which can be added (such as calcium blockers or diuretics)
Patient which suffers stroke have a good feed back to calcium blockers so in my opinion it should be added ( moxonidine or lercandipine)
I would be more helpful if you provide me the report or images of brain MR
Your husband should be treated for high blood pressure as well as for dislypidemia (if he has high cholesterol or triglycerides) and should absolutely take baby aspirin or klopidogrel
Discuss with cardiologist or neurologist about my opinion
Give him a tablet of lercandipine because blood pressure should allowed to be more than 160 mmhg
Hope I have been of help
Don't hesitate to contact me for further questions
Stay healthy

Brief Answer:
Add treatment

Detailed Answer:
Dear
I understand you are concerned but after changing treatment your husband blood pressure will be normal.
He is taking beta blockers (carvedilol) so calcium blockers are to be added (lercandipine) and between lisinopril and losartan one of them should be taken (in my opinion lisinopril)
If he still suffers high blood pressure diuretics need to be added ( hydrochlortiazid or moduretic)
Remember to discuss with doctor about aspirin
Hoping that my advise will help in normalizing the blood pressure of your husband
Best regards
