Question: doctor what is the treatment of sustained lower back pain. Following is the MRI Report (central disc protrusion at L5S1)

Hi, I had gone through your question and understand your concerns.
First stages of lower back pain, without leg pain or significant symptoms ( difficulty walking, sleeping and working, urinating problems) are treated best with a couple of days rest, NSAID drugs and when pain subsides, physical therapy.
This needs to be correlated with imaging findings and in your case disc protrusion correlates with first stage of disc herniation.
Remember not to lift heavy weights, and not to bend over excessively.

Dr thank u for ur quick response, but I was prescribed ARTROTEC 75 for one month once dayly but another physician advice my not the for one but I should take it for ten days and break for five days and continue for another ten days because it sametimes affect kidney pls dr what is your advice thank u

Dr I was prescribed ATHROTEC 75 FOR ONE MONTH ONCE DAILY but another physician advice also that I should not take for the one month instead I should take for ten days and break for five days then I should continue for another ten days for the one month because it affect kidney then dr what is your opinion

Hi and thanks for asking again.
Arthrotec is a combination medication that contains Diclofenac ( NSAID) and Misoprostol ( used to protect your stomach from possible Diclofenac damage).

Side effects of Arthrotec include side effects of Diclofenac and Misoprostol and one of them consists in rarely affecting kidney function.

Best thing to do is to control your symptoms with the more possible minimal dosage of the drug.

Kidney function tests help in deciding the correct dosage and frequency of the Arthrotec.

If you control your symptoms with taking Arthrotec periodically, this is the best thing for you.

However if you don't control your symptoms sufficiently, I don't any problem in using Arthrotec daily.

Side effects related to kidney function usually happen in previously damaged kidney function, using other drugs etc.

If Lyrica is improving significantly your symptoms, it's ok use it, bearing in mind possible side effects of it, however Lyrica ( Pregabalin) usually is started in advanced stages of neuropathic pain.

Usually starting treatment consists on NSAID drugs.

Care must be taken when Lyrica is stopped, this should be done gradually.

Dr understand some of d details lyrica is in form of capsule so when stoping how do I stop and in my case what are possible side effect and advice me on proper medication of my symptoms

Lyrica is used single dosage for 5-7 days and the dosage could be raised if there are not satisfactory results.
