Going by description of symptoms, possibility of inflammation of stoma needs to be considered. Exact diagnosis will depend on clinical examination.

1. Is the Stoma functioning well? ( Is there stools coming out adequately from stoma?)
2. Is there redness in skin surrounding stoma?
3. Do you have associated pain in abdomen?
4. Is there any bulge surrounding stoma?

Possibility of Peristomal Herniation needs to be considered

Dear Ms
Going by the description provided it is likely to peristomal excoriation. Possibility of parastomal hernia also needs to be considered if there is bulging of area surrounding the stoma especially during the act like coughing which increases intrabdominal pressure.
I would suggest you the following
1. Local application of barrier ointment like Dr XXXXXXX Zinc oxide paste surrounding the stoma where there is excoriation will help relive the inflammation. Also Coloplast paste can also be used
2. Antispasmodic like Drotavarin will provide relief from abdominal pain.
3. Thorough clinical examination from physician to rule out Peristomal Herniation
