Suggest Treatment For Swollen Ankles And Shortness Of Breath While Having Pulmonary Edema
Question: my father aged 80 he is having pulmonary edema and he is taking dytor 20mg twice a day. is it enough for him or sahll we give lasix along with above medication? because he is having swekking on his ankles and shortness of breath sometime like he is physically active.

Hello ,
Dytor and lasix are the same medication and replacing one with another is not going to help .
Rather dytor is a more advanced congener of lasix and has better absorption characteristics as compared to lasix .
By your history what you have is called diuretic resistance .
Here adding tab zytanix 2.5 mg once a day along with Dytor may help . However electrolyte should be monitored.
Other cause of swelling like decrease hemoglobin / albumin should be ruled out and corrected .
Any medication change should be done only after consultation with your nephrologist and under his monitoring .

Though adding zytanix is a next logical step ,
However at 80 years there can be major electrolyte imbalance in an occasional patient with CKD .
As i don't know the clinical profile of your father all medications he is on , How significant is the edema , Is there any pulmonary congestion .
Please do discuss it with your doctor before giving any medication .
Also close monitoring of his daily weight , urine output is important.
Also 3 days zytanix would be a temporary solution. A discussion with his doctor regarding long term diuretic plan would be helpful . Also the plan for management of his CKD or the heart ( the primary cause of his edema ) would be an ideal way forward .
