It is because, such kind of lesions come up with the loss of blood supply to the area and is very common in diabetics. It may be the start of Gangrene that is a condition in which tissues get dead due to loss of blood supply.
Yes if it really is this gangrenous lesion (Can be decided on reviewing the pictures) on the sole of foot, you need to go straight to the Podiatrist who is a personnel trained to deal with the diabetic foot. Yes you may also go to the ER for urgent care.
