Brief Answer:
Probably varicocele

Detailed Answer:
Hello dear,welcome to HealthcareMagic forum.

You are probably suffering from a varicocele.
Here the the venous channels draining the testes become dilated and tortuous,to cause discomfort during walking and prolonged standing. On palpation,we get a "bag of worms" feeling. This disappears on lying down.
Anyhow,we need to confirm the diagnosis by a local examination. Also, there some conditions like renal lesions that can cause secondary varicocele(this is unlikely for you,as you had this swelling as such for years). We have to exclude these conditions also.
Your doctor(a general surgeon or urologist) will guide you in this. You may be ordered for an ultrasound of scrotum and abdomen.
If diagnosis of primary varicocele is confirmed, this can be treated with a simple, minor, endoscopic ligation procedure.

Hope I have answered your query.
Wishing you good health.
With regards,Dr Aslam.
