Suggest Treatment For Symptoms Of Bipolar Disorder

Question: Hello doctor, I am looking for some behavior related answers about someone known to me. The lady in question some times just loose control over herself and get very angry bu assuming things which are not true in general. Once she go in this state, she just thinks negative and bad and behavior changes like others or very close relations also do not exist for her. When she is in such a state she is totally different personality. After say 1 or 2 days she gets normal and repent a lot promising she will not do anything such ever after. She also says that she just could not control her anger even though she knows what she is doing is wrong. The thing that do not help is she thinks only negative once angry and put zero efforts towards coming out of such situations. Her mom died when she was just 4 and had troubled childhood because of various family problems. Could be a case of anger management as well, I am not sure if this case is for a Psychologist or psychiatrist. Any guidance in this regard will be of great help.

2. was there any other major trauma in her life ?
3. Is there episodes of bouts of happiness and excitement followed by anger ?
4. how is her behaviour generally? socialises well? any problems at work?
repeated change of jobs ?
5. does she like being isolated ? any eating disorders ?

Thanks for the response doctor. Here are response:

1. She was only 4 years old when her mother died, so basically she do not remember anything.

2. After her mother passed away, her father got depressed and bit disoriented. She is youngest and has 2 elder brothers and 1 elder sister. So education wise she started bit late, and upbringing wise they have not see very good people around or a good family atmosphere. There is not other trauma as far as I know. She says her father and brother did not allow her lots of freedom when she became adult. But as far as I know it was due to the love marriage of her elder sister, they were protective of her. But still I feel she was not allowed to be free.

3. Yes she get happy also very easily, mostly her behavior is like a child. But not particularly after anger. She gets too excited listening something and just follow that thing without thinking good or bad, and if you try to explain however politely some times she takes it like refusal of her will and gets angry, or at some later point may think over it and get angry.

4. She is child like mostly, she is very good otherwise. She is a homemaker, and socializes bit less mostly in that case I feel she has inferiority complex. But she develops a negative image about someone and then do not like to meet them. But if she likes someone she starts believing them a lot and get influenced very easily.

5. When she is angry she isolate herself, like will sleep and do not talk and remain inside the room most of the time. At that time she only thinks negative and aggravate the situation and takes decision which are like senseless, or without thinking even her good or bad. And later even repent that she just says those thing in anger and would not do in actual. At that time she do not even think if any of her step will hurt her near and dear ones. But later regret a lot, that because of her every one is suffering.

Hello

Thanks for getting back with such detail. It looks like this person has bipolar disorder. This is how you should go about it

1. Avoid situations which are known to trigger her anger
2. Rehabilitation in terms of asking her to take up a hobby or interest apart from the family. Trying to get her into a less demanding job will also help
3. Behavioral therapy in terms of anger management will certainly help. Someone professional or a group therapy will be the best.
4. The best person for this would be a psychologist first who will take time and give her CBD ( cognitive behavioural therapy), that will be the first step
5. If CBD does not help the psychologist will then refer to a psychiatrist for mood stabilizers.

Dr Vijayanthi in my hospital (M S Ramaiah Hospital, Mathikere) is a good psychiatrist and I highly recommend her. She also does CBT. Unfortunately I do not know any psychologist in your part of the town.
