Suggest Treatment For Tear Of Labrum
this are findings in a right shoulder mri probably caused by repetition stocking shelves for 20 years i am a 43 year old man
i was given a cortisone shot which has not help
my question is should i have a surgery

If the tear of labrum is a full thickness one then surgical repair is recommended. Labrum is not very well vascularized so the spontaneous healing is slow. Steroid shots will only help in reducing the inflammation. Healing needs complete rest for at least 4 weeks. There many be thinning due to cyst and that can interfere with normal healing process. The pain will persist till it heals properly.
It is better to get it arthroscopically repaired. It will help in early recovery.

If the tear is more than partial thickness tear then surgical treatment is recommended.

I have seen the attached report. It is not likely to be a complete thickness tear. Surgery can be avoided if complete rest is provided to the shoulder joint.
The pain from shoulder can radiate to the upper chest due to associated muscle spasm. Taking oral muscle relaxants will help.
