Suggest Treatment For Tinnitus, Tingling Feeling Sensation In Left Ear And Dizziness

Question: Hello,my name is XXXX.I am 23 years old.I am having tinnitus since 5-6 years which is not a problem for me.Around starting of 2015 i was having tingling sensation in my left ear like someone is poking my ear with a thin piece of grass.I visited the ENT he said that I have dry wax in my ear which might be the problem.He gave me eardrop for 2 days and after 2 days he cleaned that.Still the tingling sensation didn't stop so I revsited him and told him about that and I even told him about my tinnitus so he advised me PTA test of audiology and report came normal and he told me that there is no problem with ear.After that around 9 XXXXXXX 2015 i started feeling mild dizziness and a little pain at the base of my head so I went to a GP and he gave me some medicines for 3 days which didn't helped me so next I visited a neurologist and I told him about my neck pain and my '24 HOURS MILD DIZZINESS'.He told me have an X-ray of the cervical area and after seeing the report he said that the problem is in the muscles of neck and gave me some medicines and told me to go for physiotherapy for 7 days and revisit after that.The physiotherapists are doing SWD and TENS for me and I don't think that it has given me much relief for my mild dizziness,although it helps a little in my neck and trapezius pain but not much.Now apart from all time 24 hours mild dizziness, I have started feeling a little pain in my left ear(my right ear is undeveloped and closed so no problem in that), head and little in jaw which is not always, just occasionally which I told my physiotherapist and they said that its trapezius pain which is doing it and I will feel relief with the ongoing physiotherapy treatment.Now today was my 4th day of physiotherapy and I'm still not feeling much relief for my mild dizziness which is ongoing 24 hours and is the main thing that irrirates me and apart from dizziness everything else doesnt bothers me much,just a little.
So please guide me what to do.

I understand your query and concern.

The combination of tinnitus and dizziness usually points to inner ear disease.

Its very unlikely for the muscular reasons to be the cause in your case.

The cause of associated pain in the left ear,head and jaw has to be carefully evaluated.

The cervical spine has to be carefully evaluated through MRI spine,since x ray will not provide sensitive results.

Additionally Rotating chair test and Computerised Dynamic Posturography/CDP will provide additional sensitive details.

I also request you to give a complete list of medications you earlier used.

Treatment largely depends on the outcome of the results.

I am having tinnitus from 2005 and I've adjusted my life according to it and I don't think my problem is because of this.I rarely feel Pain in my ear and when I feel Pain its not in the ear itself its beneath it.I believe in sternocleidomastoid muscle.I mainly feel Pain at base of skull and In upper trapezius muscle.I use to sit on computer for long hours and when used to do weight lifting I usually felt strain on my neck and my ongoing physiotherapist told me that my poor posture have caused this.

Ruling out Primary causes is more important than supplementary causes.

Detailed Answer:
Hi Mam.

Muscular disorders rarely contribute to tinnitus.

This phenomenon is more unlikely in cases of long standing tinnitus(since 2005) as in your case.

Long standing tinnitus usually has underlying vestibular disorders.

Computer work for long hours can lead to muscular discomfort but not tinnitus.

Tinnitus should always be looked as a separate entity.

The tests I mentioned in my earlier answer has great sensitivity to diagnose both postural as well as internal ear causes.

I strongly recommend you to rule out these disorders first,before attributing them to benign conditions like muscular or postural causes.

In addition a short course of Tab betahistine/Vertin 4 mg under medical prescription once a day at bedtime will help to control this tinnitus and associated dizziness.
