Suggest Treatment For Tunica Albugenia Injury

Question: Dear Doctor, in 2013 I sustained penile injury during the intercourse with my ex-girlfriend as she clamped my penis hardly when I was inside her (I felt like a needle-type sensation, which was really painful). Moreover, I experienced Candida which was treated with strong medication in Hospital. Especially, for the first few months I had a lot of pain in my penis and the treatments I was given for that were not effective at all. Then I mostly used a topic substance such as the Tea Tree one in order for me to feel better and the erection had improved too. The other day I was checking my penis and I could notice that there is a very small lump in the area where the pain is. I cannot withstand this pain no more as it also gives me problems during intercourse and it goes up to my nerves. Please, could you help me with this matter? I am looking forward to hearing from you. Thanks for your query ,based on the facts that you have posted it appears that you have sustained injury to Tunica Albugenia a sheath that envelops both erectile bodies called Corpora Cavernosa subsequently a scar tissue has formed at the site of injury which is causing pain in penis and painful intercourse .

I shall be happy if you can post me few more details to make precise comments on your problem .
1) Was there swelling and bruising of penis immediately after injury?
2) Have you noticed curvature of the penis on erection?
3) Is there any difficulty in penetrating vagina?

The pain and lump at the site of injury is due to scar tissue resulting as a normal process of healing .

Please consult qualified Urologist for clinical examination and get following basic test done to confirm the diagnosis.
1) Ultrasound Scanning of Penis

Further treatment will depend upon result of these tests and final diagnosis.

Many thanks for answering to my query.

At the stage of the injury, the penis had been clamped hardly for a moment (something that I never experienced before and I forced myself not to shout to
avoid complications with my ex-girlfriend). Following that, I did not notice any change of shape and/or bruising immediately after the injury. I am aware that the problem or pain comes from a certain area of the penis (nearer the penile tip)
in the exact point where the penis was clamped or hardly compressed and where the small lump is. This often affects my normal erection and I feel so demotivated before having intercourse. Moreover, the pain may vary according to the stage of erection and the intercourse situation. I noticed that once, after manipulating the lump softly, the pain had gone for a few days and came back. Depending on the situation, I may experience pain during intercourse and it is not enjoyable at all. At this present time, I follow proper personal hygiene and drink cranberry juice which helps to keep the urethra healthier. I saw a couple of urologists after the injury, but they would rather investigate other than the penis itself (such as cistoscopy which I did not agree as they wanted to check my bladder). The dermatologist said my penile tip is clean. But I can notice some dermatitis which comes and goes frequently (it is probably due to the skin friction from having sex). Once, I decided to have the ultrasound scanning of the penis done, but the few radiologists I spoke don't have the necessary equipment to carry out the test. They said that they would need a particular probe connected to the device. So I stopped looking for that in order to learn the real reason of the pain in my penis.

Thanks for follow up .It is very difficult for any clinician to diagnose internal injury of penis only on physical examination .

Ultrasound Scanning of penis will highlight precise nature of injury ,scar etc .

Look out for other diagnostic centers where the fcility is available .
