Suggest Treatment For Ulcers After Helicobacter Pylori Infection
I also have acid reflux which I work to control with diet. All H2 blockers and PPIs ALWAYS give me more acid (I don't use them). Liquid antacids such as Gaviscon and liquid Rolaids help.
Seven days after finishing the antibiotics most of my pain had come back. I then had the H. pylori breath test and went back on 500 mg Clarithromycin and 1000 mg Amoxicillin for two more weeks.
I'm going to have a scope done next week. My scope done thirteen months ago showed no H. pylori or Barrett's, and irritation from acid reflux. If the scope shows an ulcer, what treatment do you recommend?
I do not smoke or drink.
Thank you VERY much for your help!
try other regimens like sequential therapy or quadruple therapy
Detailed Answer:
Hi,
Welcome to HCM.
Thank you for the query.
I understand your concern.
First blood test is not a reliable test to check for H pylori, but if breath test is positive then its pretty sure that you have the infection.
Also if the infection persist even after two weeks of therapy then rather than repeating the same course its worthwhile trying other regimens like sequential therapy or quadruple therapy for H Pylori after consulting your gastroenterologist.
But what I feel is your symptoms may not be due to H Pylori at all, it might be an innocent bystander especially if endoscopy is normal.
The medications for sequential therapy include ( amoxicillin, levofloxacin and PPI) and quadruple therapy is a bismuth based regime.
Also if scope shows ulcer and H Pylori positive then you will need these therapies and biopsy from ulcer if in stomach.
In addition lifestyle modification will help you.
1) Eating on time
2) Taking adequate sleep
3) Regular walking
4) Avoiding sleep 2 hours after meals
5) Cutting down on stress and anxiety
Getting some basic tests like doing complete blood count, thyroid function test, stool examination will help.
All this will help you to control symptoms in long run.
Hope this answers your query.
Wish you a speedy recovery.
Happy to help.
Hi, good
Detailed Answer:
Hello,
Thank you for the query.
good to know your breath test is negative.
Endoscopy is unlikely to show any ulcer.
If at all it shows any ulcer then need to see where it is.
If in duodenum then may need to treat you with double dose of PPI like esmoprazole and recheck it after 8 weeks.
If it is in stomach then need to take biopsies form it and start double dose PPI.
Some times its worthwhile to check serum gastrin levels in presence of recurrent ulcer.
Hope this answers your query.
Best wishes.
