Question: 63yo male, Fall/landing railroad tie left mid back. 3"x5 swollen tender area@site day 2. Mild tenderness LUOQ abd. Rebound neg. c/o pain with deep breath/cough. Neg dizziness; neg bld in urine;VSS? ?? r/o rib fx, kidney trauma, spleen trauma?? should he be seen ED today?

I understand your concern

Pain with deep breath/cough is mainly due to the injury.
But for safer side, I would suggest to consult the physician and undergo chest x-ray and USG scan to rule out any damage in the internal organ.
Take following advises:
Tablet DICLAC-PLUS three times a day after meal
Tablet RANTAC three times a day
Apply warm pad application
Wear elastic crepe bandage to support the breathing.
Do not lie in the affected side
Take bed rest for 4-6 days
