Suggest Treatment For Urethral Pain Caused By Retrograde Ejaculation

Posted on Fri, 22 Jan 2016
Question: I have a foley XXXXXXX up my urethra. I just had a wet dream last night and ejaculated. It was very painful and still hurts. Only a small amount came out of my urethra. I feel like there is extreme pressure down in that region. My XXXXXXX still takes my urine, but I know the ejaculate did not go into the XXXXXXX as it bypasses the prostate. I am very concerned... What are the risks? I don't have this XXXXXXX removed for another 3 days. Please help me!!! I'm so scared. Thank you

Brief Answer:
This is due to what is called as Retrograde Ejaculation

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you have been catheterized and self retaining indwelling catheter has been placed in your bladder through urethra .

You had a wet dream and have noticed very little semen being ejacukated by the side of the catheter and have discomfort urethra .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) What was the reason for putting an self retaining catheter ?
2) Were you suspected to have narrowing of urethra (Stricture urethra)?
3) Was an endoscopic procedure (VIU) done to incise the stricture ?

During normal process of ejaculation bladder neck gets closed and urethral sphincter open hence semn enters inforward in to urethra and being ejaculated through normal urethra opening .

Where as whenever there is catheter is in urethra bladder neck remains open resulting into semen to move backwards and enters in to bladder to get mixed with urine .what is called as retrograde ejaculation .

The pain and discomfort that you have in urethra may be due to narrowing of urethra .

Do not worry this will get resolved and you will have normal ejaculation through normal urethra opening after removing the catheter .

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

Brief Answer:
Urine leak is due to spasm of the bladder .

Detailed Answer:
Hello
Thanks for follow up .
Urine leak from urethra around the catheter is due to spasm (Involuntary contractions )of the bladder secondary to foreign body sensation in delicate urethra .

Take medication like Oxybutinine twice daily. It will get resolved after removal of catheter .

Thanks and Regards.

I could jot at the time, however now afyer they have closed, i have to urinate very very badly and qhen i try to take a warm shower, water leaks around my XXXXXXX and burns very badly. The on call soctor told my dad to go ahead and remove it. If it is removed and i still cannot urinate, should i go back to the ER and have another put in? Why am i urinating around the XXXXXXX through my urethra is i have a XXXXXXX in? Wouldnt it take all the urine? I really need an swift answer on this. Thanks. xxxxxxxxxx

Brief Answer:
Leakage of urine around the catheter is due to bladder spasm

Detailed Answer:
Hello
Thanks for follow up .Since you have a indwelling which drains out urine continuously and there is hardly any urine in your bladder .
It is very common to get leakage of small quantity of urine around the bladder and it is due to Spasm of the bladder .

Taking Oxybutinine twice daily will help to reduce the spasms of the bladder and prevent leakage .
