Suggest Treatment For Urinary Fungal Infection
Question: how to treat male urinay fungal infection... It followed a antibiotic treatment for bacterial urinary infection'
Several treatments with 3 consecutive days of diflucan cause great relief which starts to deteriorate gradually after 15 days. Patient is 75 year old. long term diabetic normally satisfactorily controlled. Urine culture is negative but symptoms keep reappearing:
- urgency, frequency, some incontinence,
- oral thrush,
- fits of sneezing with mega nasal mucus and sore teary eyes, mucus in loose stools,
- spiking glucose tests,
- toe nail deterioration, skin rash,
- lose stools with mucus
Patient has prostate ca., treated with IBR in 1994
Patient is diabetic treated with oral meds. since 1983

Hi,
Long term not properly controlled diabetes, along with malignancy is a strong risk for developing these kind of resistant fungal infections. Apart from diflucan there are other higher antifungal agents which should be very judiciously used under strict supervision of Urolgist as these may carry risk of some side effects. A culture and sensitivity test repetition would be helpful.

I should have mentionned that recent culture was negative. Wouldn't an infection which affects the whole body indicate a candidiasis which as I read in Merck, 2013 review on urinary candidiasis, that a longer course (14 day) of medecin is called for?
I will have to convince my urologist, and for that reason I am in search of some indication that I am on the right path. This whole situation has been going on since march 2015.

I am really sorry for delayed reply. As you mentioned "Wouldn't an infection which affects the whole body indicate a candidiasis"-
This is not true always. There are many other similar infections. Blood culture would help than urine culture as symptoms are generalized now.
Culture is negative but you still have symptoms- so I think there is need to rule out alternative causes. Discuss this with your Urologist.
