Question: I am experiencing vaginal dryness and incontinence. I have tried birth control pills, estrace, a
dilation of my urethra, antibiotics, and vesicare. I have no period unless I don't take my birth control and have not experienced mood swings that are different from normal. I have experienced hot flashes and my urethra was dilated to help prevent cystitis and to prevent urine retention. I have XXXXXXX syndrome and depression and am taking birth control, zoloft, and cranberry pills.

Welcome to HCM.

You are facing the problem of vaginal dryness and urinary incontinence. You also suffer from XXXXXXX syndrome and depression.

May I please tell you that very few women with XXXXXXX Syndrome menstruate spontaneously.
Estrogen therapy is required for a regular shedding of the uterine lining ("withdrawal bleeding") to prevent its overgrowth. Withdrawal bleeding can be induced monthly, like menstruation, or less often, usually every three months, if the person desires.

I suggest you continue taking birth control pills, you can simultaneously use estrogen cream for local vaginal application.

For urinary incontinence, I have some suggestions for you:
1. Practice kiegel's exercises to strengthening pelvic floor muscles.
2. Estrogen cream will also help in incontinence.
3. Avoid constipation. Take fibrous foods. Reduce weight if you feel that your weight is more.
4. Since your take antidepressant zoloft, I would suggest its replacement with anti-depressant which has anticholinergic effects as well, like imipramine or amitriptyline (prescription drugs). Or add-on anticholinergics can be tried like,oxybutynin.

Kindly consult your gynecologist and discuss these issues. Do not make any drug changes on your own and without your doctor's guidance.
