Suggest Treatment For Vaginal Infection And Over Active Bladder Syndrome

Question: hello,

i am a 29 year old female, got married in XXXXXXX this year, april end i was diagnosed with uti which was cured after 5 amikacin iv was admistered (1 g each day), again in may end i was showing symptoms of uti and after the culture reports were done the same treatment was given, this time for 7 days, i am now free of the infection, the culture reports have come back negative two times, i had a vaginal infection too, for which i had treatment (flucanazole, satrogyl and vaginal tabs for 6 days), the hvs culture is also clear of any infection, however the frequent urge to urinate persists even after infections are gone, on consulting a urologist, i was told it is post uti cystitis, i am on soliten 5 mg and rivotril 0.5 mg, though the frequency has gone down a bit , it is still there, please help me , how long will this take to solve , i have been told not to dwell on it, it is causing me depression as well,

Thanks for your query ,based on the facts that you have posted it appears that you have what is called as Over Active Bladder syndrome(OAB).

Normally one gets sensation to pass urine when bladder gets filled up with 300-350 ml of urine however in OAB there are involuntary contractions of bladder even when it gets filled up with small quantity (75-100 ml) of urine and these involuntary contraction of the bladder creates urge to pass urine with a very small quantity of urine being passed .

The primary object in treatment of OAB is to suppress these involuntary contractions of bladder which is accomplished by anti muscuranic drugs like either Solifenacin to be taken twice daily or Darifenacin once daily .Your Dr has rightly started you Solifenacin .Take it twice daily .

It takes 2-3 weeks to see the results of these drugs and has to be taken for 3-4 months .

It is not a lifetime disease .It is always secondary to chronic irritation of bladder due to repeated episodes of UTI .However one can have recurrence of OAB after having repeated episodes of UTI which is very common in females .

Sexual activities do not play an role in this disease .hence sexual activities will not aggravate it further .

thank you sir,

one last question, can you give me a time frame as to how long it can last? i know it is difficult to say so, but just that my normal daily life has been disrupted and also can it be interstitial cystitis?

One thing I can assure you that it is not Interstitial Cystitis which is characterized by an agonizing pain while passing urine .

It normally takes 3-4 months to get cured .

sir,

you had answered my questions a few days back, frequent urination and the persistent feeling has led me to the doors of a psychiatrist, because that is all what i think of day and night, he has diagnosed depression, and prescribed pexcep cr 12.5 mg, risdovia 0.5 mg, you are a XXXXXXX doctor, please help me understand , if these meds will take a toll on my body, i am a newly wed and only 29 years old, also will these meds have any effect on my bladder problem?

There is no harm in taking anti depressive drugs prescribed by Psychiatrist for short duration along with Darifenacin once daily .

It will not have any side effect except slight dizzies .

Burning urination with pungent smell with no signs of UTI can be due to concentrated urine due to drinking less water .

Ensure to drink more water so that you pass 1.5 to 2 liters of urine in 24 hours.

sir,

drinking more water leads to frequent urination, this seems like a never ending cycle, is there something that is not being treated, it has been over two months in this condition. my ultrasound reports and creatine reports are all normal

Please start the treatment (Darifenacin nce daily and observe for improvement after 2 weeks .You need not worry about frequency due to increased water intake .Your concern is urgency that is more important .The frequency will automaticallt go down once urgency is controlled .(reduced)
