Suggest Treatment For Venous Insufficiency In The Legs

Question: Dear Doctors of HealthCareMagic
After getting answers from dermatologists, I would kindly ask a surgeon with knowledge on vascular medicine and microcirculation for an opinion as this finding troubles me a lot.

The palms of my hands get a kind of mottled-red appearance. The redness is markedly pronunced especially in my left hand. Right hand is also mottled but much less red. This is never seen when I lay in bed, rarely seen indoors and mostly seen outdoors when I walk. It goes back pretty much to normal when I sit for a couple of minutes or raise my hands above heart level. I may had it for a long time butbecame worried only recently. It may be aggravated by warmer weater. I don't have any reticular changes on any other part of the body. I was wondering if it is livedo reticularis or Bier spots or some palmar erythema just my natural circulation pattern or some disease of the microcirculation. I am mostly scared of a summer presentation of acrocyanosis due to diseases that come with secondary acrocyanosis. I had very brief attacks of vasoconstriction in colder climate (blue nailfold, visible blue vessels in fingers, colder fingertips) but (according to my doctors) not to the extent of symptomatic acrocyanosis or Raynaud phenomenon. I have freshly diagnosed venous insufficiency in my legs (by dr Chandrakar of healthcaremagic and fully confirmed by my doctors at home). I was fairly obese (BMI 43 by Autumn 2015) and I've lost weight to BMI 33 currently.It may be worth of note, that these red parts (and whole fingers) are fully blanchable when touched.

Thanks for the question.
I am a Surgeon and a Vascular Specialist.
I read your query and saw the attached pictures.
I dont think there is anything wrong with your hands. It is a normal appearance of the hand even if there could be some erythema which can depend upon your Hemoglobin concentration. As far as the decrease with raising of limbs, that is also a normal happening as the Subclavian artery gets compressed on limb elevation and the flow decreases to either limbs. I suggest do not worry about it and do not seek any medical help for this issue.
If you want clear yourself of any Vascular pathology leading to this then you will need a Color Doppler of the arterial system of both upper limbs, but that will also not be required if the Blood pressure is equal or nearly equal in both your limbs. As far as Acrocyanosis or Raynauds is concerned, I dont think it is either of those or you could develop either of these conditions.
I suggest you relax and do not make much of this as this will not trouble you at all.
