Suggest Treatment For Vomiting And Fever In Child

Question: Hi, my little one, 9 months 24 days, weight - 9kgs above, started vomiting on 4 days back followed up with fever of 100.5. Vomiting subsided with ondem 2 days back but fever has persisted. Doctor suggested crocin first but then as fever kept coming back we shifted to combiflam about 2 days back. Combiflam is helping bring down temp from 102 (peak) to normal 97.5 ! Can movement be of 4.5 deg F? Do you suggest anti biotics should be started? Post hearing Routine stool test result, our doctor said today its a viral and continue with only combiflam medicines and fever should subside in 48hours naturally. Second opinion needed.

I have gone carefully through the details provided by you. Your child seems to be suffering from a gastroenteritis. It could have a bacterial or viral cause. But generally the condition is self correcting and does not need to be treated with antibiotics, unless associated with bloody diarrhea. Fever relates to infection and action of combiflam seems satisfactory. Give tepid sponging along.

It is good to hear that the stool test does not indicate any bacterial infection. Antibiotics are not needed. Give him plenty of fluids to drink. The adequacy of the hydration would be evident from the frequency of passage of urine. If that seems to be fine, you need not worry. Also give some of curd or yogurt. It contains plenty of beneficial bacteria.

It may take another few days to recover (may be more than 48 hours) but I do not think there is anything to worry.

However, if the child develops lethargy or temperature goes higher and fever lasts for more than 7-10 days from the beginning, I would suggest you to visit your doctor and get the blood tested.

hi again,

1) I have attached his routine stool report of yesterday evening. Please advise on the same with details. Eg. What is occult blood - in traces ? Would also like to disclose that hes on formula milk of Nan-pro 2 - brand nestle.

2) By which day can we expect for his night temp range to drop instead of increasing as currently? He went from 100.5 ranges to 102.5 range of last 2 days.

3) Is room AC okay in this heat? We live in XXXXXXX He's developed little cold since last night and sneezes occasionally but no congestion is apparent

4) Could this fever be related to teething? Next to molar teeth one is crowning.

1. I have gone through the stool report and it seems fine. I would not worry about the occult blood since there is no XXXXXXX blood. Pus call count is also OK.
2. Fever may be there for few more days. Hard to say till when. Only if fever lasts for more than 7 days I will get concerned.
3. Fever is not likely to be related to teething. Get orthodontist opinion on the crowding without delay.

Thanks for the response. Fever went away 2 days back, yet:

1) he has flatulence on and off.

2) his stool report , in reaction had mention of acidic'. What does that refer to?
I was diagnosed with lactose intolerance 2 years back. What's the precise way to find if someone is lactose intolerant?
3) would you recommend the child have celerac, which is a nestle packaged food as one or two of his meals? He does also have other home food.
4) he's only had formula milk and hardly any mothers milk. Please comment on the same.

Lactose intolerance may be secondary or congenital

Thanks XXXXXXX for writing back. Sorry for not being able to write back earlier.
1. Flatulence is caused due to overgrowth of intestinal bacteria which degrade the undigested carbohydrates and produce gases along with acids which makes the stool acidic as in the report. It could be transient or due to lactose intolerance.
2. The presence of reducing substances in stool
as well as hydrogen breath test is generally used for diagnosis of lactose intolerance but confirmatory test is genetic testing which is not regularly done.

3. I personally prefer home made mashed items to formula like Cerelac although they can also be given. Rice based products are more preferable to wheat based products.
4. Mother's milk is the best possible feed for a baby upto 6 months of age. But you need not bother about it at present. Just give a semisolid diet for now. Milk need not be given although you may give curd.
