I am 4w6d I had light spotting when I wipped yesterday light cramps only lasted an hr went to er counts were 143 did ultrasound didn't see anything but all the bleeding and cramps have stopped breasts sore today took a test this morning still positive just wondering when I will start bleeding if I miss carried my periods are always normal 28 days
Brief Answer:
??Threatened abortion, please visit obstetrician for progesterone
Detailed Answer:
Hello ma'am and welcome.
Thank you for writing to us.
I have gone through your query with diligence and would like you to know that I am here to help. Bleeding/spotting at your gestational age should be investigated immediately, and as the ER would have done that, confirming viability of the pregnancy is a good thing. The diagnosis should most probably be a threatened abortion (which puts your pregnancy at risk), but a miscarriage can be avoided with some simple measures like:
+Initiation of progesterone (which should support the pregnancy)
+Bed rest for while
+Regular/more frequent pre-natal visits
+hCG injections may also be required at your stage
I would thus request you to meet an obstetrician who would help support your pregnancy and also carefully monitor you as required ma'am. Please do not hesitate to write to me for any further clarifications, I am always here to help.
Best wishes.

Brief Answer:
An ultrasound at 5 weeks may be unreliable
Detailed Answer:
Hello once again.
It is usually really difficult to visualise and confirm or rule out pregnancy at 5 weeks, so we cannot completely rely on the ultrasound at this point. But an ultrasound anytime after the 6th week would provide us with more accurate information.
Unfortunately we will have to wait for the ultrasound, but if you wish to retain your pregnancy then I would strongly recommend a visit to your obstetrician:
+Check hCG levels and get hCG injections at the earliest
+Along with other recommendation listed in my previous response
As you have not mentioned it, in case you have requested for an abortion and have taken medication for the same, then you can wait for a day or two longer, and then revisit your doctor for a repeat course, as in some cases more than 1 course of the abortion pills may be required. A urine hCG or serum hCG level can also help in confirming or ruling out pregnancy, especially when we have a previously recorded value.
I hope this helps ma'am.
Best wishes.
