Suggest Treatment For White Patch On Neck

Question: my 8 year son has recently got one white patch about 1 cm round on the neck. seems it is vitilgo. Also few very small discoloration of skin over eyebrow. have seen doctor he has suggested Dicaris 50 & topograph cream. any secondd opinion pls

I have gone through your query and I have also seen the photograph.

This seems like Vitiligo.
The topical preparation contains tacrolimus in an ointment base. I agree with this choice of treatment.
It is one of the topical treatment for focal vitiligo. It should be applied twice daily; gently rubbed in with finger tip.
Tacrolimus can either be used alone Or it can be combined with topical moderately potent steroids e.g Mometasone furoate.
Response is usually evident within 6-8 weeks, with twice daily applications of tacrolimus.
Oral tablets contain levamisole, which has an immunomodulatory role in slowly spreading vitiligo. It is usually give an weekend therapy on 2 consecutive days(Saturday and Sunday).

Hi.

Vitiligo is an unpredictable disease. The patches usually re-pigment in 3-4 months.
It may or may not recur; there is no way to predict its future course.
Treatment of vitiligo is guided by the extent of involvement and whether it is spreading or stable.
While topical treatment alone suffices for focal patches such as this; for widespread vitiligo systemic treatment is required.
Topical treatment options are topical steroid, topical tacrolimus, basic fibroblast growth factor etc
Surgical measures can be tried in stable vitiligo if medicines do not produce the desired results and include suction blister therapy; melanocyte culture and grafting etc
Having said that, limited disease has a better prognosis than widespread or extensive disease.
Since your child has just a few patches, the treatment that he has been prescribed is apt for it.
Subsequently treatment can be modified (added or substituted) if need be.

Systemic treatment for widespread involvement includes phototherapy, oral steroids etc.

1) by any chance the medicine prescribed have some major side effect or can by any chance even aggravate the more spots

2) Dietary restriction
a) Sea food
b) fresh water fish
c) Red meat ( mutton)
d) Chicken
e) egg
f) Dosa
g) Dry fruits
h) calcium & iron syrups
i) Citrus fruit
j) Banana / apple
k) Chinese (occasionally) ( concern is msg)
l) curd
m) Butter
n) XXXXXXX ghee

3) what in your view, is the timeline we sud start seeing results

4) Do we need to avoid sunlight

Hi

-No, there are no side effects with this treatment. Some patients may complain of a slight smarting sensation or burning sensation with tacrolimus but this is usually not bothersome.
-No dietary restrictions
-Usually 6-8 weeks when you should start noticing an improvement
-Strict sun avoidance is not advisable but since these patches are devoid of melanin therefore strong sun might cause them to turn red.
