Yes, the pain could be from keloids.
Keloids, particularly at joints may impair mobility at joints by not only restricting free movement at joints because of the hard, tough nature of keloid scar but also because of pain from stretching. Alternatively, pain on movement could also be due to an unrelated problem e.g a joint Or a musculoskeletal problem
Keloids are formed from excessive scar tissue that may follow tissue injury which may be either from an accident Or surgery Or following a burn. Keloids are firm to hard, raised above skin surface, often extending beyond the area of initial trauma Or wound and may either itch Or pain. Keloids may also start growing after years of inactivity and may itch or pain.

I have reviewed the Image. The scar looks quite extensive, extending over the shoulder region right upto mid-arm.
Pain during movement could be either because of stretching of taut scar tissue Or it could also be due to shoulder joint degeneration because of lack of full range of movements, at this particular joint for so many years. At the moment I would suggest only symptomatic treatment e.g a tablet of co-codamol (low dose codeine and paracetamol). However, if the pain persists I would suggest you to also review with an orthopedician Or physiotherapist for underlying joint/ musculoskeletal cause for pain.
