Suggest Ttreatment For Burning Sensation In The Genital Area

Question: ok I want to get one more opinion by a gynecologist im trying to figure out what I have going on in my genital area which has persistent feeling that lets of heat consistently kinda of a burning sensation I guess but like when I wear jeans I can feel the heat through my jeans like im on fire and these red spots on my groin bikini area just keep coming and won't go away the last time I had sex was six months ago I cant really remember when all of this started happening after being so busy with my job and all and since that my menstrual cyle has been messed up instead of 5 to 6 days of my period t only last like 2 days n will start off dark.brown blood I will attach pictures so you xan see im just really worried im just tired of feeling uncomfortable all the time the area does not itch and I dont hace any discharge besided wheb I start my period and it atarts of brown

I am Dr. Abraham, OB - GYN and would be glad to assist you with your query.
1. Let us first talk about the sensation of constant burning and heat on fire kind of sensation that you mention.
THat generally occurs due to some kind of infection in the genital area.
Now, you have no foul smelling discharge or itching, so a serious vaginal infection is ruled out.
2. You also mention the red spots and they are visible in the pictures too.
They are recurrent, and keep on coming over the same area.
They do not resemble herpes or any other STD.
They are in all probably suggestive of - folliculitis (follicle - hair follicle and itis - inflammation ).
Folliculitis means infection of the hair follicles over this area, this is quite common.
It maybe triggered by various factors , I will list a few -
- Unprotected sex with a new partner
- Abrasion during rough sex/ masturbation
- Shaving
- Change in products that are used locally eg. soap / gel / cleanser / douche / intimate cleaning brands that are used rampantly nowadays / toilet paper / tampon or sanitary napkin etc.
- Long nails injuring the area
- Soiled / damp undergarments
- Swimming in a communal pool
- Public toilets that are not very clean.
3. The folliculitis can be treated with local antibiotic cream, containing Neosporin, Bacitracin, Fusidin, Betadin etc.
If they do not respond to this, oral antibiotics are required, but then you need to consult your OB - GYN for this, as they are prescription medications.
An OB - GYN can also carry out a vaginal swab to rule out any possible infection.
4. SOme general tips to deal with this -
Use the ointments as described.
Avoid touching or handling the area
Have protected sex or none at all till this resolves
CHange underpants frequently - use only cotton ones.
Avoid any kind of product to clean the area, just clean with lukewarm water and XXXXXXX dry with a gentle towel.
Take vitamin C supplements to speed up healing
4. Regarding your menstrual issues, folliculitis generally has no connection with menstrual disorders.
You could have an internal exam, Pap smear, pelvic ultrasound and hormonal levels to know the cause of such a disturbance, if you do book an appointment with your OB - GYN.
Sometimes , far more common reasons can cause such a change in your menstrual pattern - stress, anxiety, late nights, travelling, change in weight, unaccustomed exercise, smoking, drinking, eating erratically , being on hormones / pills of any kind etc.

sometimes I feel like I have an odor but a few months ago I was in a hot tub with about 7 girls these red spots started happening after that I belive now the burning sensations all the time started happening after I had sex last summer I have not had sex.since then I dont wanna start dating anyone until I kno whats going on with my body it doea get itchy from time to time but nothing major I was tested for hepatitis a b and c chlamydia gonorrhea and hiv by primary care doc all negative I cant remember if he tested for.hsv or not when I have no cloths on all I feel relief from the heat penetration that my vulva puts on through my cloths is so uncomfortable I have not shaved in awhile because im just worried n dont kno what to do I have been stressed out gained weight n sometimes my whole body feela like its overwhelming heated when I sit down I can feel it all the way through my legs at times to or my whole body im just freaking out its just so embarrassing and depressing ima send a few more pics i have looked in the inside of my lips and there is no red spots like this I guess im just worried I may have hsv of some type sorry that im freaking out its just a scary scary thing I feel like im the only one this is happening too I feel like I never wanna have sex with anyone ever again I also mention that I have eczema on hands an feet and could cobtact dermatitis be ruled out

You are so young, and very healthy.
This does not look like any kind of STD.
This definitely looks like folliculitis.
You just need to approach an OB GYN and have the workup that I suggested.
Am sure oral antibiotics will take care of these red spots.
After that you can definitely have sexual contact normally.
Regarding the contact dermatitis, only a dermatologist could examine the lesions and tell you if that is the case.
