Brief Answer:
Reduce streangth of pathces,Buprenorphine or methadone can help

Detailed Answer:
Hello,

Thanks for writing to us.

I can understand your concern. Fentanyl is an opioid drug. After using it for long ,it is difficult to stop without medical supervision and help.Codeine is very less potent opioid and it can not replace effects of fentanyl.

You can have two options.

1. You can reduce dose of fentanyl patches and then gradually frequency of application of patches very slowly as per your treating doctor's advice. In this procedure , if you feel withdrawal,it can be treated with burenophine or methadone.

2. If you want you can stop cold turkey. You need to ask your doctor to prescribe sufficient dose of burenorphine or methadone. You can gradually decrease the dose of same according to doctor's guidance as these medicines are with less dependent potential and can be stopped easily compared to fentanyl.May your doctor treat you in hospital for initial 3-4 days to prevent severe withdrawal.

Hope I have answered your query,I will be happy to help further.

Regards,
Dr.Chintan Solanki.
