Question: Had a gastric sleeve operation 11 dec 2013 .. Was 109 kg. lost till 64 kgs then in last few months went back to 69kg
Im worried i need someone to help advice me how to lose weight and why im gainjng weight again..

My food habit is the same before and after my operation i didnt change anything its only the quantity be ause of the operation that changed to less..

For example i used to eat one and a half mcdonalds burger with half the soda and all the fries

Now i eat half or three quarter of kids meal cheeseburger and half or less of the kids meal frise and quarter of the soda..

I used to eat 3 pcs of pizza with fries and 2 pcs garlic bread and salad
Now i eat one small pcs of pizza and just taste the salad and no more than 4-6 pcs potato wedges

Regarding the pasta i used to fill my plate with 3-4 big ladel scops
Now less than a cup of yoghurt i put pasta and i dont finish it most of the time

Ofcourse whats in the pasta will make the different sometimes its filled with sauce so i eat less becsuse i get filled up quickly

So im wating much less and i lost all the weight with the same food habbit

I dont know why now im gaining weight

Also i noticed something after the operation i eat chocolae and sweets more than before
Before i was a fast food addict i disnt eat much chocolate or sweets
Only after the operation i became addicted to chocolate and sweets i feel it is easier to digest and doesnt make me uncomfortable after eating it like savory food

By the way i didnt have any complications after the operation and maybe 5 times i vomite because at the begining i forgot when i drink water so quickly that i made an operation so i throw up my food and that happened maybe 5 times from my mistake

But nothing much happened

So can u please give me a diet that i can do to lose weight

Im afraid to gain my weight

And surprised why im gaining now i didnt change anything my food is the same as when i was losing weight same bad food habits

I cant change it because i dont like lots of food like sea food and vegetables i only eat lettuse cabbage potato parsley cauliflower but fried not in oven .. Carrots fresh not cooked .. Thats all the vegetables that i can eat.. Nothing else im ok with fruites but i dont eat them i dont know why.. I eat chicken breast only .. Crispy most of the time but ok with grilled.. Meat i eat burger and kebab .. Im nit in to meats without a starch bread or pasta or even rice.. I dont like it plain..

Brief Answer:
XXXX, your problem is your metabolism along with poor food choices

Detailed Answer:
Hi XXXX, I have reviewed your very informative query and yes, I know exactly what your problem is as I have dealt with it with many of my weight-loss patients.
I truly want you to be successful. You have been honest with me, but the facts are you are going to have to make some permanent changes in your overall lifestyle to remain successful in keeping your weight off, your energy up and to achieve balance in your health. Here we go.

You cut down your portion sizes after the surgery and lowered your calorie intake overall and the surgery was successful. Your body has adapted to your reduction in calorie intake by dropping your metabolism so that you are gaining weight on the calories that previously allowed you to lose and maintain your weight. That is coupled with a craving for sugar, chocolate, sweets. So, I imagine you are eating more calories because of the sugar cravings.

The next thing I noticed is that I don't see any evidence of nutritional teaching prior to or after your surgery as you are still eating junk. You still eat McDonalds with fries and burgers full of dead energy, saturated fat, chemicals which work against health. You can get on a healthy diet, but you seem so adamant about what you will and won't eat. If you won't do it and aren't committed, you will not be succcessful. Please try to commit. These are general guidelines that will make a difference: 1) Deal with food addiction - no more sugar, chocolate. This may very well be related to a change in brain chemicals. Depending on what else is going on in your life, you may need an addiction therapist, anti-depressant therapy, or referral to another health professional who specializes in addiction medicine; 2) No more fried foods. You can bread things and bake them, but no frying. More saturated oils and fat. Chicken is good, kebab is good, burgers should be lean with fat squeezed out and drained on paper towel and - why so many food aversions? Have you ever had an eating disorder? Thinking about these things will help you understand the problems that need to be treated that go beyond diet in order for a diet to be successful.

3)You did not mention exercise. Part of a diet/lifestyle plan is to include activity even if it is just walking daily. I need you to exercise to get your resting metabolism up.

4) Can you drink green tea? Green tea will also help elevate your metabolism. I suggest 6 oz. three times per day with or without caffeine.

5) Try to stick to low glycemic carbohydrates. You can get a list off the computer by looking them up. Your body is having sugar issues so staying on low glycemic carbohydrates will help that.

6) Using a small amount of honey is better than any other form of sugar as recent studies have shown it to help lose weight used in small amounts and in place of other sweeteners.

This is a lot to digest. If you would like clarifications or more information, please write back. If not, feel free to close out the question. Thank you for coming to HCM - Kathy Shattler, MS,RDN

Thank you doctor for your reply..

I appreciate it..

Regarding any kind of sport i was a computer person always spending my time on my computer ..

I never comit to any kind of sport or routine .. I lost weight very slow maybe thats a blessing that i dont have saggy body after losing 40 kgs..

Let me mention that the main reason that i made the operation is that o dont eat healthy not because i dont want to its because most of the healthy food i dont like them.. I made alot of diets before my operation and wasnt able to continue because i end up not eating anything the whole day and only drinking juice because i prefer not to eat better than eating something i dont like.. My body cant accept i will throw it up if i force myself to eat it even if i had to.. And i dont force myself to throw it just happens .. Thats why i struggled my life trying to find healthy food that i can eat and was always yoto dieting because i fail then search for something else.. And so on but i never had an issue with too much food what i ate was normal to eat one sandwich with appetisers sometimes extra half sandwich but its not like what i see the fat people eat they eat a whole box of pizza which i couldnt do before .. And 2-3 whole meals..

Its just that my food kind is so fatty and horrible for the body i totaly understand that
But i dont know what to do

If i ever see 70 kg on the scale i know that i will see 80 then 90 then back to over 100 and i cant bare to have this number again

I will try my best to commit to walking at least half hour a day

And will bake my food not fry it..

But regarding chocolate and sweet
I eat about 2-4 mix kind of chocolates each day sometimes cake its like the size of muffin but its still carbs and sugar i know..
And as u said i understand that my body now wants a change because it adjusted to my current calories intake

But is it ok to have at least 1 chocolate or desert in small portion as a snack per day?

And can u plz give me a diet that i can try for one week and see if i can lose weight

I promise to listen to what you say

and i'll keep a journal of what i eat

Even if u want me to quite eating that one chocolate

Also i forget to mention something

Sometimes i feel dizzy and want to faint so i rung and drink XXXXXXX nectar juice its the XXXXXXX size 125 ml

But mentaly its a saver for me so i keep one on my table beaide my bed for such thing when it happens

Before i dont drink juice only soda and i never finish half even when i was fat before the operation

But now as i mentioned i always have craving for sugar

I feel it gives me power and opens my eyes when i feel tired and dizzy ..

And its so easier to eat sugary food than savory i dont know why

Brief Answer:
XXXX I lost the answer I just wrote

Detailed Answer:
Here we go again

I am giving you guidelines so you can put your own diet together. Remember we need to focus on raising your metabolism. You have agreed to the exercise. Now, you must drink hot green tea three times per day to increase metabolism.

Are you drinking dairy? If not please supplement with 800 mg of chewable calcium with vitamin D. Also take a chewable multivitamin please. Take the two separately from each other.

Your sugar cravings may be because of something called reward deficiency syndrome related to dopamine and serotonin dysregulation and is especially prevalent in families with problems with alcohol or it may just be a result of a carbohydrate-type addiction.

Chew foods slowly and please follow these rules: AVOID: nuts/seeds, CABBAGE, popcorn, dried fruits, CARBONATED Beverages, Granola, Stringy vegetables like celery, broccoli, corn and cabbage, fried foods and breads.

Drink liquids in between meals but not at the same time as the meal. A meal can only consist of 4 oz. of food at a time or the sleeve can be stretched and the surgery compromised.

Bake or grill your chicken, lean beef patties. For a fried like flavor you may take skinless chicken and marinate it in low fat ranch dressing overnight and then get some low calorie flavored chips and crush finely. Put the chicken in the breading and bake until done.

So, utilizing these guidelines your breakfast lunch and dinner should all contain no more than 1/2 cup of total food. Two small snacks in between and your small treat at night.

XXXX, do you think this will work? Oh and get your blood sugar checked please.

Thanks and you can still write back if you have more questions. Thanks Kathy Shattler, MS,RDN

Brief Answer:
You requested a sample menu and your problem may go beyond sugar cravings

Detailed Answer:
Hello, I am so glad that you wrote back. I wasn't done working with you. Let me give you a sample menu. Also, please give exact values when we ask for a lab value such as blood sugar. I specifically wanted to know a number as I suspect you are dropping low in your blood sugar which is contributing to your dizziness and perhapse your cravings. We need to focus on keeping your food intake regular. Also when you eat the sugar you crave it will start a roller-coaster ride with your blood sugar causing rapid shifts between higher and lower blood sugars thus contributing to more symptoms like dizziness and sugar cravings. Again, this can go along with dumping syndrome.

Sample Menu:
6 0z. of hot green tea
30 minutes later
1 chewable multivitamin
3 ounces yogurt and 1 ounce of drained fruit mixed together
(Concept - do not eat more than 4 ounces at a meal and no liquids with meals)

mid-day snack
1/2 cup fresh carrots with fat free dip

lunch
6 oz. hot green tea
30 minutes later you may have food
2oz protein (i.e. 2 oz. grilled meat)
2 oz. of starch( 2 slices of thin bread)
1 fat- mayonnaise or Miracle Whip

mid-day

4 oz. of skim milk with cocoa powder in it sweetened with Stevia ( may need to use lactaid milk if milk give you gas or stomach upset)

Supper
6 oz. of green tea

30 minutes later
chewable calcium/vitamin D
2 ounces of potato with margarine
2 ounces of protein (chicken, sauteed lean beef strips, cottage cheese, etc)

Night-time snack

4 oz. of low fat milk with cocoa and Stevia

Later-small chocolate muffin

I cannot answer a separate query on another patient, my dear. I can say that if her insulin is high she needs to begin following the principles of a diabetic diet as she will in all probability develop it if she doesn't care for it now.

Thank you so much for coming to me at HCM. Keep me apprised of your progress and further problems. Do not let your blood sugar slide up and down. If it gets below 60 drink your juice and call the doctor the very next day to explain your need to treat feelings of low blood sugar. Best wishes. XXXXXXX Shattler, MS,RDN
